Communicating in an Emergency: Sample Fire Evacuation Instructions

In case of fire evacuation I need help. Please read the instructions below.
- I need someone to stay with me.
- To move my wheelchair you need to disengage the brake.
- If I'm on the ground floor, push me to the nearest exit and assist me safely out of the building.
- If I'm on a higher floor, we need to wait at the nearest designated safe exit. Please note the stairwell and floor numbers.
- You need to call 911 for me. My cell phone is in one of the side pockets on my knapsack at the back of my wheelchair. Press 1 to turn on the power. Wait until it is on. Dial 911. Press button with the telephone symbol to send.
Please indicate:
- You are with [my name] who has a disability, cannot speak and uses a wheelchair.
- Give the location - stairwell and floor number.
- Tell them you are waiting for evacuation out of the building.
If the phone does not work, tell someone exiting the building about my location and to contact the fire department.
- If it is safe, wait with me for evacuation by the fire department.
- Instructions to Fire Department:
- My name is...
- I cannot speak.
- I weigh approximately ___lbs.
- To lift and evacuate me:
- slide my tray off
- unfasten my seat belt
- you can use a one or two person lift
- I can be laid down if necessary.
IMPORTANT: DO NOT LIFT ME IN MY WHEELCHAIR DOWN STAIRS.
